AuthoraCare Collective, a nonprofit hospice, palliative and home-based care provider serving central North Carolina, and Inferenz today announced their ongoing collaboration to build data and AI capabilities. The work is designed to strengthen patient engagement, support referral workflows, and improve operational decision-making.
The foundation is a secure, connected data platform built on Snowflake. It serves as a single source of truth across two EMRs, a patient triaging system, a state health information exchange and HR data, with a Master Patient Index that creates one unified record per patient. Dashboards across seven domains, including patient safety, quality of care, and business development and revenue, now give leadership timely insights that previously required manual reporting.
On top of that foundation, the teams are developing enhanced patient engagement experiences and AI-assisted referral workflows to help patients and families reach services faster and free care teams to spend more time on direct patient support. All clinical and admission decisions remain with qualified healthcare professionals.
