Inferenz has released its Risk Analyzer and Next Best Action AI Agent, a new addition to the Caregence™ platform built for healthcare providers. The agent sits on top of the risk-scoring platform a provider already runs, reads across clinical and operational signals, and tells the care team exactly why a patient’s risk score fired. A prioritized action plan follows within seconds of the flag.
It pulls patient data from the EMR, claims, scheduling, assessments, labs, and remote monitoring devices, then reads it across five risk categories at once: clinical, utilization, behavioral, social, and financial.
High-risk patients get a structured PDF with ranked risk drivers and an immediate action plan; low- and medium-risk patients get a preventive monitoring summary instead. The agent also touches measures worth up to 63% of a provider’s HHVBP score.
